Workers’ Compensation Claims

Due to the Texas Department of Insurance, Division of Workers’ Compensation regulations, it is recommended the ‘Employer’s First Report of Injury or Illness” form DWC-1 be reported directly to the insurance carrier.  For your convenience, we have attached a DWC-1 form for printing purposes only.  Please be aware that several carriers offer online workers’ compensation claim reporting as an option.  Wortham’s claim representatives are available to assist you should you have any questions and/or problems.
